    Settings > General > Restrictions.

You can stop all downloads there, or restrict this one app, but it has to be on the device to hide it.

Both. When you restrict an app, it is hidden from everyone's view, so it can't be used, but because it's already on your device, it can't be downloaded, either.

You should really consider blocking any and all purchases from the App Store.

    Both.

Setting Age based restrictions will hide the App if it's already on the iPhone, and will prevent downloading such apps from the App store if they are not already on it.

You can of course simply delete Instagram from the iPhone completely, and then setup the Age restrictions to prevent any Apps with age ratings higher than those you approve to be downloaded or even be accessible on the device.

Use Restrictions on your iPhone, iPad, and iPod touch - Apple Support

Instagram for instance has a 12+ Age rating. Set the Age restrictions below that, and Instagram will be disabled from running if its on the iPhone (it will be hidden), and be prevented from downloading to the iPhone if its not.

AAll downloaderd has to be approved. Problem is when it has been approved it's impossible to "unapprove" it

Why not simply delete it from the iPhone. and the next time it requests for download Approval: deny it.

To delete an App press and hold on the App until they start to shake, then press the on the corner to delete it.

If you have disabled the deletion of Apps in Restrictions, you will need to re-enable it for a moment while you delete the App.

you dont think that has been tested? If it has been approved it looks like ITS impossible to unapprove it.

Then set the restrictions as explained above so it cannot be downloaded again. Or if it's kept on the device it cannot be used and will be hidden.
