Linking loyalty/membership cards to credit cards

When Apple Pay was first announced, it was stated that it would be possible to link loyalty/membership cards to credit cards so that one merchant transaction would access both, for convenience. So far I see no way to do that. I'm in Canada, have added my credit cards to Apple Wallet, and have added my Air Miles card to Apple Wallet. How do I link them? Or does the merchant and/or bank have to do something on their end to enable it?

Kohl's recently became the first retailer to offer one-tap checkout, enabling customers to both pay and earn loyalty points via Apple Pay with a single tap.

How is this actually done... does the customer have to create a Kohl's account? How does Kohl's associate a credit card with a loyalty card customer? The customer could use any one of a number of credit cards, for example.

The arrangement applies to payments made via Apple Pay using Kohl's Charge (the retailer's store card), also crediting loyalty points under the Yes2You Rewards scheme.
