Q: Canadian and the W-8BEN Form for selling on iBooks?

Hi. As a Canadian do I need to fill out a W-8BEN form or a SS-4 form to sell on iBooks? I don't know if I need a ITIN or EIN number to complete my US Tax section on iTunes Connect. Specifically do I need to fill out sections 5, 6, 9 and 10 on the attached form?
