You will need to use a device like the Apple Watch or any other which can sync to Health to add your activity there.
You can also add manually going to the Health app > Fitness > Walking + Running Distance > Add Data Point
To make your activity recorded by your device, you need to enable it in Settings > Privacy > Motion & Fitness > Fitness Tracking turned on

I have answered you how to enable health monitor in your iPhone and how to add manually activities not recorded i.e.: if you run without your iPhone to register it.
- You can also add manually going to the Health app > Fitness > Walking + Running Distance > Add Data Point
- To make your activity recorded by your device, you need to enable it in Settings > Privacy > Motion & Fitness > Fitness Tracking turned on

Once you have done both, do some activity and make sure your iPhone records it. In case it does not, get back with more detailed information.
