iTunes 12 can't detect my iPhone 4S

My iPhone 4S is in almost perfect condition and is not 2nd hand, it has never been tampered with and is running iOS 9.3.2 with no problems except from it won't connect to iTunes. It used to work on my current PC and no changes have been made since the last use but it has not worked for a month now, I have tried everything I can think of to make it work, including reinstalling iTunes and buying a new charger. When running a diagnostic check on iTunes everything seems to be fine with iTunes just not connecting to my phone.

If you haven't tried it already, maybe this will help:

If iTunes doesn't recognize your iPhone, iPad, or iPod - Apple Support

You said you tried a different charger. How about a different lightning cable? Another USB port?

As step 5 in the above link states: Unplug all USB accessories from your computer except for your device. Try each USB port to see if one works. Then try a different Apple USB cable.

Have you tried syncing over WI-FI?
