Q: Verizon iPhone 4 stuck at Authorization Required

Thanks for your reply/question. They punted to Apple when several in-store Verizon attempts to help (including new SIMM) failed.

I brought my phone -- always believed it was a 4; IMEI ****; Apple JUST told me it's a 5S -- to Verizon store in Washington DC two weeks ago and they said they replaced the SMM card (when I repeated this to local Verizon store they merely nodded). I get "Activation Required" when I touch the screen, swipe the slide-to-unlock button, and, also, about ten seconds after I successfully sign in to apple ID w username and password, the phone (which looks normal for a few seconds) reverts to lock screen and "Activation Required."

...and it has been working on Verizon just fine for three years.
