Q: Long time Problems with Podcasts App

I listen to Podcasts mostly in my car.  I prefer to use the "media interface" because it will charge my phone and provides better podcast information than using BlueTooth.  For a long time a number of issues have been building and they are getting to the point of not only being majorly annoying but making it dangerous to interact with while I'm driving.

System information:  iPhone 6s Plus running 9.3.2 (though this problem dates back to a previous iPhone 5 and started around the introduction of iOS 8

Auto:  2012 Mercedes ML350 with the 30 pin cable connected with the extend 30 pin to Lightning connector (yes I got it work kind of).  Neither Mercedes or a 3rd party offer the lightning connector for that cable set up.

Issues:

1)  Even with Podcasts App open, music app not open.  If I plug the phone in, most of the time it will play the first song on my first playlist (I'm really getting sick of that Blues song).  Work around:  I must start the Podcasts App playing the podcast before plugging it in.  If I unplug use the phone for any application and leaving the Podcasts app open all the time it will still go to that first song on the first playlists.

2) Probably not tied to Podcasts app (but doesn't happen with Music app), one out of 10 times plugging in I get a high pitched squeal.  This is corrected by unplugging and plugging back in.

3) I like to listen to my podcasts in oldest to newest on a per podcast basis.  I'm often playing catchup and listening to several in a row.  I finally got that figured out how to work.  However, when it has played the last episode for that podcast it immediately starts playing song one of playlist one (drat that Blues song again!).

1 and 3 starting on the iPhone 5s and back with iOS8.

2 started more recently.

Since 9.1, new problems have cropped up:

4)  In the past if I had a podcast going I could push the button and ask Siri a question.  Now, most of the time, I must ask Siri questions and give her commands when the phone is not plugged in to the lightning cable.  Using Siri will set the phone back to default of playing 1st song on first playlist again in Music when plugged back in.

5) In the past, when I would received a phone call, the podcast was paused, while the phone call was going on.  Now, the podcast continues playing in the background and I have to unplug is from the lightning connector for it to stop playing.Using phone will set the phone back to default of playing 1st song on first playlist again in Music when plugged back in.

I don't use the phone with a speaker system to comment if it occurs then as well or only with the car, but I'm wondering if others are seeing these same issues and have work arounds that are as dangerous for me to implement when driving.

Why don't i just use BT?  I have no idea under BT where I am at in the podcast.  Even over BT, phone will start playing first song in first playlist once all podcasts under a single title have been played.
