If you have an app on your phone then somebody has bought/downloaded it on the account that you are using on it - if you have 'automatic downloads' enabled on your phone then it might have been bought on a different device or on a computer (does anybody else have access to your account and/or devices/computers ? Have you changed your account's password ?)

If you've contacted iTunes Support (how did you contact them ?), and it's been more than 48 hours since you did so (and you've checked the email account's spam folder as well as the inbox ?), then contact them again.
