I recenetly purchased ATV4 and noticed the volume when streaming my iTunes library is very low. Once i start a movie or show i can barely hear it, I have to exit and go back to menu and then restart the exact show or movie and the volume is now back to normal.
Any ideas how to fix this?
Try restarting the Apple TV, if that doesn't help you might try a restore.

Do you have any sound effects enabled in the iTunes preferences playback preferences or indeed on the Apple TV.
Not to my knowledge... I have checked a lot the settings and seems to always default back to low volume... Until i stop it exit a start the movie or show again.

Yes I have the same or similar issue. Mine happens more on the showtime anytime app, if I pause the show and restart the volume is muffled or lower. If I rewind 10 sec or back out of the show, replay the volume is back to normal or louder. Waiting on tv os 10 update, if it doesn't fix the bug I'll return or exchange it.
Update on my post above TV OS 10 fixed the issue I was having with the volume.
