If you take a screenshot and it does not appear, it could be a problem with the screen itself. If you do not see it, then try taking a photo of the screen while it is on with another device, so we can see the problem you are describing.
Beat me to adding it here. Try the screenshot I recommended to see if it appears there.
If it is a screen problem, you can make an appointment at the Genius Bar of the nearest Apple Store to have the device examined by Apple, and based on their findings, they can provide you your options.

A screenshot is made directly from the video feed, so if a problem does not show there, then it helps to narrow it down to the screen itself. You may be able to just get the screen replaced at a lower cost than having to replace the entire phone.
