Q: was my iPad hacked?

I was on my ipad when it locked and said I needed to reactivate it because the find my phone app had been activated. I only have the find my phone app on my ipad. Does that mean someone has access to my Apple account?

iPad (4th gen) Wi-Fi, iOS 9.3.4

Is it locked now? Go to iCloud.com from a browser on any computer and log on to your account. Check to see the status of your devices and for good measure change your Apple ID password. If anything looks amiss follow the steps here:

Hi. This may have just been due to this If you can't activate your iPhone, iPad, or iPod touch after installing iOS 9.3 - Apple Support activation requirement following an iOS update.
