can you hear them clearly. maybe it's them.

You can back up the phone in itunes and reset it to factory stettings

Did this just start or was it always the case? Are you on speaker phone? Do you put them on speakerphone? Who are you calling? Are you on a cellular network? Have you contacted your cellular network provider? Are you getting a good signal? Are you calling over wi-fi? Are they answering over wi-fi? Where are they? Who is their ISP? How are they taking the call? Are they on a phone or using software? Have you contacted Apple? Have you contacted them on your phone? Can Apple hear you clearly if you call them?

Take it to an Apple store for diagnosis. Make an appointment first.
