Is an iphone's image file folder structure (e.g., /mobile/Media/DCIM/112APPLE/) the same on both the iphone and in the icloud backup? What would happen if you renamed the folder structure in icloud, or is that even possible?

There isn't a way to access the iCloud backup in that way. It can only be used to restore to an iOS device or to set up a new iOS device.

Sorry but I cannot clarify that. To the best of my knowledge there is no way to view the structure or content of an iCloud backup. You can view the date and the size of the complete backup but nothing else.
