Try updating to the latest iTunes which is 12.4.3.

If it does not let you you may need to update your Mac's OS, the latest is 10.11.6.

It may be a hardware problem, like the cord, or possibly the iPhone. Try to isolate the problem, like plugging a separate device into the computer.

Alternatively you can try to use a different USB input, sometime data exchange pins may be broken on the port, but very unlikely.

How does updating iTunes help, this is not vey likely an iTunes issue.

On occasion iTunes has hiccups, although it may be unlikely, it is still a possible solution.

If you are having issues with iTunes on your computer the very first thing to try is updating iTunes to the latest version, 12.4.3.
