Once that passcode is forgotten, the only way to fix the issue is to restore the phone. Download and install iTunes 12.4 from iTunes.com. Connect the phone to the computer with the charger cord. Hold down the home button and the power button until the iTunes logo appears. (It will show the Apple, then the iTunes) The computer will offer the option to update or restore. Choose restore. When the process is complete, the phone screen will say Hello. Set up as a new phone.
