Forgot my iCloud password and have been locked out,how can I reset my password .?

I Cannot access my iCloud because I forgot my password and security questions.It keeps saying I'm locked out.

HOw can I reset my password ?

iPad 2, iOS 9.3

Enter your Apple ID and click Forgot Password.
