Why do replacement iPhones have limited guarantee period
In what circumstances is an iPhone supplied with less than 12 months guarantee please/

Warranties is 3 months or the rest of your previous iPhone warranty.
It depends which is longer.

If it were an Insurance Claim replacement could one expect a new 12 month guarantee please?
No, but then we users here are not Apple employees.

Apple doesn't do insurance. You should ask that question of whoever handles your phone insurance.
