iPhone 5 shutting down, showing wrong time, etc.

Every day for the past three, I have had issues with my phone. Day 1 - showing 8:30 PM when it was 9:45 PM

Day 2 - when plugged in to charge, shut down on it's own Day 3 - when fully charged and plugged in shut down and I had to enter Apple id/password to get int ... any thoughts?
