Cannot remove CarbonX app

Hi everyone .

I have an annoying problem ,I have jailbroken my previous Iphone 4s and i have had this "carbonX" app stuck on it ever since .Unfortunately after i got my new iPhone 6s and used the back up from my previous phone and the app somehow got stuck on my new one as well. Every time I restart or update the phone it reappears even though i have deleted it a thousand times .Any suggestions ?
