That "report" is completely meaningless. No 3rd party has legitimate access to any of that information.
It means nothing. I saw the report.
How do I fix my phone? I want to use it with a SIM card in my country?!

If it is carrier locked, only the carrier it is locked to can authorize unlocking it. Contact them. Find out what their policy on unlocking is and whether your phone is eligible to be unlocked and pay any applicable charges.
