locked out of my iPad

I received activation lock message on my iPad and i can not recall info i used to set up iPad I need id and password I am the only one that has owned this iPad

See either this -- If your Apple ID is locked - Apple Support OR Turn off Find My iPhone Activation Lock - Apple Support
