Need help deciphering panic report

My Mac Pro has had a few kernel panics in the past week and at least a couple of times wouldn't boot past the logo screen. I was able to get it to boot by resetting PRAM only to have it panic again. I don't know much about reading panic reports, the only thing I noticed is that it's not showing DIMM 3 or 4. Is this a RAM hardware issue? Any help would be appreciated.

Yes, very likely a memory problem when more than one bank reporting errors.
