Thanks for using the Apple Support Communities! After reviewing your post, I am understanding that you have LED flash enabled under Accessibility, but do not see the feature working when you receive a Notification. I definitely know how helpful the Accessibility features can be, and want to make sure yours are all working as expected!
I've got a few troubleshooting steps I can recommend. Before beginning, I suggest creating a backup of your iPhone using either iCloud or iTunes, as detailed here: How to back up your iPhone, iPad, or iPod touch
Once you have your backup finished, try the following steps, in order. Test after each step, stopping when the issue is resolved.- Restart your iPhone, following the steps here: Restart your iPhone, iPad, or iPod touch
- Reset the settings on your iPhone under Settings > General > Reset > Reset All Settings. This will not remove any of your personal data (like contacts, pictures, Calendar events, etc) but it will revert customizable settings (like wallpaper and display brightness, ringtones and alert sounds, Date & Time settings, etc) back to their factory defaults. It will also remove saved Wi-Fi networks and passwords, as well as any previously used APN or VPN settings.

After the restore is complete, set your iPhone up as a new device (meaning without your backup, at least temporarily). You can sign into iCloud, iMessage, FaceTime, and iTunes and App Store. Then, turn on the LED Flash setting in Accessibility and test.
