1. I have no customization - no custom ringtones etc.
2. Rebooting the phone works but is not a practical solution.
3. A "hard reset" has been suggested in earlier posts. Can somebody define a "hard reset"?
4. This is happening with my iPhone 6S.
5. No Bluetooth is turned on for anything.
6. The sound stops coming out of both the ear piece (tested by playing music) as well as the headphones. Its not related to calls only.
7. In my case an additional data point may be the repeated removal and insertion of headphones - I typically remove the headphones unless I get a call. This has never been an issue in prior releases.
8. It is 99.999% a software issue.

Oct 18, 2016 6:32 AM in response to akbhargavaby Mr G13,Hard reboot is essentially erasing your iPhone and restoring it from your backup. It works but the good effects last only for a few days. The problem comes back again!
We need an update to fix this asap!

Default ringtones, hard reset, soft reset, change earpods, in-out earpods, set-up-as-new, reboots....
Nothing seem to work. After a couple of days/calls, listen to a few songs through earpods the problem appears. And then nothing work, no call audio, no earpods audio, no video sound, voice memo does not record voice...
The last thing i tried is to
Disable "Allow Call on Other Devices" (Setting--> Phone) and Reboot (not format)
It's the second day of trial and still no issue. I'm testing it thorough so i'll get back with more soon.I 've already sent a bug report to Apple because i'm positive it's a software bug. But... my iphone 6s has the Samsung processor and Not the TSMC (The famous Chipgate issue).
All of you that have the same issues, what kind of processor you have? In case it's a software error compatibility on certain hardware. Perhaps you can download Lirum Device Info Lite - System Monitor or some other app to find out what kind of processor you have.
I will aggree that we have to join forces, find out the symptoms and replicate them and probably a solution that works, in order for Apple to listen and issue a fix ASAP.

Oct 18, 2016 8:55 AM in response to kouloupby philipede1,I Have done all the same fixes. This has happened to me on 3 handsets. All 6S 16Gb. there has to be a common hardware link to this bug. How can I see the manufacturer of the processor.? I looked in settings, but no mention of the processor manufacturer. This has to be the common denominator . How to tell processor ?

Oct 18, 2016 1:06 PM in response to philipede1by Puertopadre16,@Philipede1, this same issue has occurred to me on all 3 of my 6S phones and two 7 phones all at 128 GB so I would be cautious in saying that its a common hardware link however, the argument provided about the different type of processor could, again also said with caution, be causing this issue but until Apple begins to take notice and issues a fix, keep the feedback reports and bug reports coming. Further to this, as mentioned in previous workarounds, either having a custom ringtone or not will eventually cause the same problem to occur albeit not as soon.

While I was listening to music at Spotify someone calls me and I can't answer the call because I can't hear the other person. Even if I unplug the EarPods I can't listen a thing. When the other person hangs up, I tried to listen to music again but it stop working. No EarPods, no Loud Speaker.
I just can fix it by rebooting iOS.
It happend twice today. -

Clearly an issue has been identified with the current iOS software since the latest upgrade to iOS 10 and all its variants. In order to have Apple take a closer look at this issue and come up with a global fix, strength comes in numbers so the more people that report this bug the better. Lets all do our part as a community of users and submit feedback to Apple using their feedback link. http://www.apple.com/feedback/iphone.html
Unfortunately, and as frustrating as it is, until a fix has been found and issued by Apple, we are all in the same holding pattern and must make do with the temporary fixes suggested by the community of iPhone users.

Oct 19, 2016 12:19 PM in response to bndesforgesby ~psigmon~,I factory reset my 6s plus and restored from my most recent icloud backup. The problem persists. I haven't reset and set up as a new device, however, I didn't have time for that.

RIght now I am evaluating if setting the ring tone to default instead of using a custom one makes a difference...
Someone suggested it, and I am desperate. So far so good.

Oct 19, 2016 12:26 PM in response to JTh_sweby Puertopadre16,@JTh_swe, I attempted this method of using a standard issue ringtone when it was first suggested by one of the community members and it too eventually failed in time. As I mentioned before please use the link above in one of my earlier posts to submit a feedback report to Apple and be prepared to wait out until Apple finds a solution.

I have set up as new, taking nothing from backup, loaded a new IOS 10.0.2 from iTunes and added no apps other then what comes with the IOS and still does it after a few phone calls. Apple is not even acknowledgeping this problem
