I see that you are having issues with the FaceTime HD camera in your MacBook Pro. There are several things we can try for this issue.
1. Test in another user to see if it is user-specific. If you need help setting up a new user: OS X Yosemite: Set up users on your Mac
2. Check if the camera is recognized by the computer. Go to the Apple Menu > About this Mac, select "System Report" > Camera. Is the camera identified?
3. Reset SMC. Reset the System Management Controller (SMC) on your Mac
