Welcome to the Apple Support Communities. I see you're running into issues when you ask Siri to FaceTime your wife versus when you ask for her by name. I love having Siri do my bidding on my Mac so it's my pleasure to help you out here. It sounds like you've done some good troubleshooting already by reindexing Spotlight and disabling and enabling Siri.
I'd like you to test if Siri is able to FaceTime your wife in Safe Mode:Try safe mode if your Mac doesn‘t finish starting up
Ignore the title, the nice part about Safe Mode is that it only runs Apple software, so if it's third party apps or extensions that are preventing Siri from being able to FaceTime your wife, this will show us.

wife's name to being your wife?

Siri is a lady of high degree, difficult and high maintenance. She is supposed to save you time, but only after you have learned her ways which could take a few months. I ignore her.

Sep 26, 2016 5:20 PM in response to jeremy_vby buckeye351,Thank you for the suggestion. However, sound input and output are disabled in Safe Mode. Consequently, when I launch Siri, it is showing an error message saying that "No microphone is found."

Sep 26, 2016 5:29 PM in response to buckeye351by buckeye351,When I ask Siri "Who's my wife?", she correctly shows the right contact card. I have a feeling this is a serious software bug that needs to be addressed by Apple devs.

Sep 30, 2016 8:33 PM in response to buckeye351by Nima-N,Same problem here. Though the message works properly. But if I ask to FaceTime my mum or wife or any relatives, it does not and asks me to continue in the app. If I use the name instead, it makes the call.
