AutoFill not working
iMac, OS X El Capitan (10.11.4), Please help asap
You may have to delete the autofill entry and set it up again.

I loked in my contacts all info is there I even unchecked all boxes in autofill and checked them off again to see no luck.
I just tried a form and it works but leave my first name and last name blank?
I just got a partial the form in but without first and last name not shown and state not select it seems weird ,am I missing input in my contacts?
It may be the website not accepting autofill for certain fields. I've seen that before.

I'm having this same exact same issue, and it's on a page that I've used on a daily basis at work for years, and did not start until I updated to Sierra.
I've deleted the web page from the "other forms" section, and tried to reset it with no luck.
The info is for feedback form that has nothing to do with my or any personal information. I have a feedback phrase that I use, and until the update, I just chose "autofill" and it would fill the field. Since the update, I get the same message that the OP posted.
Any assistance would be appreciated.
