Try using a different Thunderbolt to HDMI adapter, loan one temporarily and see if it works. There is no reason an adapter should do what you describe.
An AJA T-tap is a different league from a run-of-the-mill tb-hdmi adapter (and priced accordingly).If all you need is connect a secondary display, it is overkill.
