OS X 10.11 file sharing issue "smb://unknown#”

setup is as follows...

I have a Lenovo running Windows 7

G5 with OS X 10.5

Mac Mini running OS X 10.11.5

I can share between the two Macs fine, and between the Lenovo and G5. I can share from the Mini to the Lenovo via smb (cmd+k [IP]) but not the Lenovo from the Mini. On the Windows machine, I can see the Mini in the sidebar and the name is "jbannMini" as it should be, but it just will not connect to it. It keeps giving me a user or network password error. All usernames/passwords/workgroups match. All machines have static IPs on LAN.

The one thing that I have noticed is that the computer name will not change no matter what I do on the Mini. It keeps showing smb://unknown#.

As you can see below, on the machine that I can share both ways the smb shows the computer's name.

so far I have repaired permissions/rebooted and disabled and re-enabled file sharing many times. I've been on Google for hours trying to find this solution. I was able to find one person with the same issue, however repairing permissions solved it for him....not me. Any input will be much appreciated.

Thank you.
