Q: How to fix a constant duplicate playlist?

I have seen other posts on duplicate playlists but none exactly fitting my issue.  None of the fixes I've tried seem to resolve it.

 

I have a single playlist that keeps getting duplicated with a "Playlist 1" name.  It seemingly pops up out of the blue first on my iphone.  I say this because when I go home and look at my imac where my local music is (and I sync via imac, not icloud) this duplicate playlist is not there until I sync.

 

I subscribe to Apple Music.  I have the icloud turned off everywhere for a long time as it kept messing with my library. 

But for grins I did create a second itunes library with no music, activated the icloud, saw a few old tracks so I deleted everything out.  Signed back into my usual itunes file, deleted the fake one I created, made sure icloud is signed out of all devices as far as Apple Music goes.

 

This change seems to have started with iOS 10 but I could be wrong about that.  It's a recent problem.

 

Have tried simply deleting it.  It returns with a day or so.

 

I have created a new playlist with a different name, copied all of the songs over to this new playlist, deleted the old playlist.  The new playlist created a duplicate within about 3 days.

 

I have tried syncing it to my computer, let both of them exist there, delete the duplicate, re-sync.  The duplicate stays gone for about a day, then returns.

 

It's always the same playlist.  I've had this playlist for over a year, this problem just started recently.

 

I don't know what more to try to get rid of this problem.  Any suggestions?
