How to Map Mouse Buttons to Page Up and Page Down for Faster Scrolling?

I use an Evoluent vertical mouse, and had used the Evoluent preference pane to map the thumb buttons to Page Up and Page Down. Sierra has disabled the preference pane.


I also use Scroll+ for fine scrolling, but without page up and page down, I don't have any options for fast scrolling. Scroll+ has a key+mouse solution for fast scrolling, but I can't use that.


I have coordination/proprioception problems, so I can't use the narrow scrollbars in os x/macos. I used to use wider scrollbars in Linux, but don't have that option in macos. I can't use tapping and need to disable most gestures to avoid accidentally triggering them. I can't use two-handed keyboard shortcuts without Sticky Keys, and not all of them work with Sticky Keys, and they aren't really shortcuts for me. I can't use keyboard+mouse shortcuts at all. I also have rsi, so I can't use scrollwheels, trackballs, etc. and can't use Apple's external trackpads.


Various sources suggest that function + up arrow and function + down arrow are supposed to work as page up and page down, but they don't work with Sticky Keys. I have an old mini keyboard with the relevant keys; I had to replace it because I kept accidentally hitting them, so if necessary I could use it for key-mapping instead of using whichever shortcuts. Either way, I'm not sure how to map the mouse buttons to the shortcuts or the keys.
