AutoFill not working after OS update

After updating from El Capitan to Sierra, Autfill will not work. The field is for feedback form that has nothing to do with my or any personal information. I have a feedback phrase that I use, and until the update, I just chose "autofill" and it would fill the field. Since the update, I get the message shown in the attached image.

"AutoFIll was unable to complete the form"

I've deleted the web page from the "other forms" section, and tried to reset it with no luck.

There was another thread in which the "Solved" answer was that "Some pages simply don't allow AutoFill". That, in this instance is not correct. This page has worked with AutoFIll for years, until the first boot of updating to Sierra.

Any assistance would be appreciated.

Hey A067. I'm having the same problem. I may have another datapoint that could prove helpful to someone...hopefully. The same page that fails to invoke autofill in Safari for Mac works perfectly in the iOS version of Safari. So, clearly, Apple messed something up with the Sierra update.

Does autofill work on this page for anyone?

Hey there, A0S67.
From your post, AutoFill in Safari isn't behaving as expected after upgrading your iMac to macOS Sierra. I rely heavily on AutoFill myself and I'll be glad to help.
Safari for Mac: If AutoFill doesn’t work in Safari -- Here is the troubleshooting AutoFill on your iMac.

macOS Sierra: Text pane of Keyboard System Preferences -- It may have been a Replace/With response.

Replace list, With list

The Replace list shows words or phrases you want to have replaced when you type them, and the With list shows the words or phrases that will be used as the replacements. To change a word or phrase, click it, then type a new word or phrase.

Sterling R,

Thank you for taking the time to help.

All of my contact info is correct. How does my contact information have any relevance to leaving a phrase on a feedback form? The feedback phrase isn't on my contact card.

AutoFill user name and Passwords is checked. I tried adding the phrase to the + form in the keyboard preferences, with no luck.

Still not working.

Neither is any AutoFill, including info related to my contact card, I have come to find.

I'm at a loss.

Thanks for confirming that, A0S67. It sounds like we need to dig deeper.
Try safe mode if your Mac doesn‘t finish starting up -- Safe mode will make sure your Mac starts up correctly. Test how Safari responds in Safe Mode. If there are no issues, reboot normally and test in your own user account.
How to test an issue in another user account on your Mac -- If the issue is also happening in safe mode, use this article to test another user account. This will demonstrate if the issue is isolated to your account, or if it's system wide.

Let us know how that goes and take care!

Ok, so I did as you suggested, and the autofill is working for my personal information in safe mode and normal boot, but not in the area I use for work. If i hadn't used it on that page every day for the past several years, and on the day I updated to Sierra, then not after that update, I'd think is was the page. This is incredibly frustrating, and I'm on the verge of wiping my system and starting over.

Thanks for replying and that additional info, it really helps! Since you still have an issue after the troubleshooting you have done, your next step before reinstalling macOS is to contact AppleCare support and they can help with additional troubleshooting that can resolve your issue.

This is still completely broken for me on all websites. I get the little icon with the face on it to drop down filling it in but it doesn't do anything
