Q: Can iTunes retain folder structure when adding documents?

I can sync a combination of folders and documents for iOS apps using iTunes. However, I can't retain folder hierarchy, meaning that no items in Mac folders get synced to the device unless I add them to the top level of the part of the iTunes window that shows the device contents for the selected app, and there appears to be no way to put documents or folders into any of the folders that appear there. I suppose this is contrary to the way file management is designed on iOS devices, but is there a way to make apps recognize a folder hierarchy through syncing to the device using iTunes?

I don't know how iTunes would do it, but you can create one (additional) level of "hierarchy" if you use your iCloud Drive.

On the Mac, open your iCloud Drive in a Finder window (Go -> iCloud Drive).  In the existing folder for the specific iOS app, create a sub-folder and name it.  I just tried this using the Pages folder.  Add documents (for that app) as desired to sub-folder.  On iOS device (running that app), on screen that shows your documents for this app, the sub-folder is shown.  This only works for one level; you cannot create a sub-folder within a sub-folder.
