Sierra install: File system verify or repair failed

Hi,

I'm trying to install Sierra on my late 2014 iMac and at the point of rebooting in the installation process I get the following error below:

When I restart it boots normally into El Capitan without any problems
I have Run the First aid from Disk Utility and that does not show any problems with the disk

any ideas?

iMac (Retina 5K, 27-inch, Late 2014), OS X El Capitan (10.11.6)

Hey Smartr,

I can understand how this would be an inconvenience when trying to have the most up to date software.

What I would suggest is using the recovery mode and use disk utility to run repair on the drive to verify it.
About macOS Recovery goes over how to use the recovery mode.

Thanks,

I had tried that before, but I release now that I ran the check on the partition rather than on the physical drive. When I run first aid on the physical drive it fails. Now I just need to work out whats wrong with the drive!

I found the problem with the disk and managed to fix it. Interestingly the version of disk utilities on my recovery partition could not fix it. However when I booted from internet recovery the disk utilities there managed to fix the underlaying issue on my fusion drive by rebuilding the link between the SSD and HD drives.
