When I am asked to enter my Apple ID and Password, I get a message "someone is trying to log in" and a map showing Madison, WI pops up. I'm logging in from my home in Utah.

Some ISP's route their traffic in unusual ways. If the egress point to the Internet from your ISP's network is in Madison, WI, your location will show as there instead of where you actually are.
