quit Safari then hold down SHIFT and launch Safari again

go to SAFARI>CLEAR HISTORY

go to SAFARI> PREFERENCES>EXTENSIONS and remove all extensions.

go to SAFARI> PREFERENCES>PRIVACY and go to MANAGE WEBSITE DATA and remove any website data.

go to SAFARI> PREFERENCES>PASSWORDS and remove passwords.

go to SAFARI> PREFERENCES>AUTOFILL and remove anything you don't want associated with the account.

As far as I know, the only way to reset Safari, so that it looked like it did out of the box, would be to restore your phone as new. You could clear history and website data, delete all bookmarks, turn off iCloud syncing, turn off Keychain, but you would still have suggestions pop up in the URL field that I assume you want to remove.

The only way to totally start over with Safari is to totally start over with the whole phone.
