Are the iPhone 6s on the apple app unlocked to any and all carriers or are they exclusively locked to just one carrier

Apple does not sell refurbished iPhones.
It uses them for replacement purposes only. The refurbished iPhone used to replace another iPhone will be exactly the same the one being replaced. So if the original one was locked to a specific carrier the refurbished unit used for replacement will be too.
