The refurbished phones sold by Apple are unlocked. This is clearly stated in the product description.

Apple does not sell refurbished iPhones.
It uses them for replacement purposes only. The refurbished iPhone used to replace another iPhone will be exactly the same the one being replaced. So if the original one was locked to a specific carrier the refurbished unit used for replacement will be too.
