Mac Pro 5.1: Sleep Failure

When the Mac Pro 5.1 is going to sleep due to inactivity or user command, then after few second, it switches off instead to remaining in waking state. It is not linked to the version of the OS (I tried 10.6.8 and the latest one).

At the next startup, the power management diagnostic contains the following Sleep failure code [code:0xFFFFFFFF0400001F]:

18:49:53.908080 +0100 powerd Entering Sleep state due to 'Software Sleep pid=108':

I would like to get additional information about this code and its meaning in order to fix this issue.

Mac Pro, macOS Sierra (10.12.2)

I also had this issue on a macpro 5.1.

By unplugging usb, ethernet etc one at a time I traced it to a faulty usb port on a third party pci card. If anything is plugged in to this port the computer won't sleep. The other three ports are fine.
