Apple ID verification loop

I tried to set up a new Apple ID for iBook publishing. I went through the entire process, including receiving and entering the 6-digit verification code. When I hit Continue it took me back to the sign-up screen and prompted for the same sign-up info.

I went through this several times but while the new verification code is apparently accepted each time, the sign-up process failed.
