iPhone and SIM keep deactivating

iPhone 6 (iOS 10.2.0) has at random times, wiped itself and deactivated the SIM card. The first time it happened, user was in Argentina and so had to be sent an activated SIM to get service. Several months later, user was in the US and phone wiped itself and deactivated the SIM. (When going through the setup on the iPhone, was given message that the SIM could not be activated). 2nd user with the company has reported the same issue on his iPhone 6. Both phones are on Verizon. Has anyone else encountered this issue?

Turing the phone off or connecting to iTunes does not work to restore service on the SIM.

It wasn't that but thank you for your reply. I called Enterprise Support at Apple and it has something to do with the Verizon profiles loaded from Apple to the phone during the first initial activation process. The profiles were not normal Verizon profiles on either of the phones, so any time the iOS updated or the phone was reset, the erroneous profiles caused Verizon's database to not recognize the SIM card and deactivate it.
