Hello,

With iOS 9 Apple built a feature that can help detect who a unknown caller may be. iOS uses mail to help figure that out.

For example if you get a email from Bob and he leaves his number. When he calls it will say "Maybe: Bob" if you haven't saved his number.

If you want to turn this of go to Settings > Contacts > Contacts found in apps, and disable it.

In addition to what Gus said, it could also be "Name Display" service. Name Display (name and number) is an extension of Call Display (number only). This is a carrier service and can vary by plan and carrier.

TBH, I am not sure how the iPhone handles these. On landline phones, if the number is in your device's addressbook, your addressbook entry will display, if not in your addressbook but name display is given, the phone will display that. If the number is not in your addressbook and name display is not given but call display is, it will display the number. Finally, it can show blocked, unknown, etc. if nothing is transmitted or pre-defined text.
