My phone battery is bulging and lifting up the screen. It is out of its warranty but will apple still do something about it? Can I still use my phone?

So I recently noticed that my iPhone 5s screen is bulging and did some research and found that it was the battery swelling. I was wondering if Apple would replace it even if its out of its warranty because the battery is defective and it is a health hazard. Also, is it still safe for me to be using it? What could happen?

Continuing to use a swollen or bulging Lithium battery is not safe. You should take it in to Apple and ask them if there is anything they can do. At the least, they can safely dispose of it for you. Apple can replace the battery for a fee.

Lithium itself is quite toxic so you don't want to handle the battery if it ruptures. Swollen batteries are also at risk of internal shorting and possible sparking a fire. Turn the Phone off, stop using it and get the battery replaced.
