MacBook Pro video problems with 4K LG monitor 27UD88-W

I have been experiencing issues with the 2 LG monitors 27UD88-W connected to my new MacBook Pro (15-inch, 2016) with their USB-C cables. When the screens goes to sleep based on the Mac's settings due to some inactivity, the monitors seem to disconnect, and when the screens come back ON after some activity, all the opened windows rearrange in random orders with some windows sometimes hidden or out of view.


The main problems is that, the Mac seems to have issues reconnecting to the 2 monitors with multiple behaviors, 1) the Mac main monitor will show a distorted screen and becomes inactive and suddenly seems to do a soft-restart, 2) sometimes one or two of the LG monitors won't reconnect, if I reset the cables they will beep but won't connect to the Mac, and if I do a hard reset I get the same response, the only way to get them to respond is to unplug the power cord and reconnect them again, this last incident occurred just today, April 13, 2017 at approximately 3:15pm ET.

Same thing here. Moreover, after my computer wakes up from sleep pretty often the resolution is changed and only possible choices in settings are 1920x1080 and then 3840x2160 @30Hz, nothing between. System Preferences for Displays also look different: instead of those little icons on "Larger Text - More Space" scale, I have just a standard list of resolutions. I have to restart the computer to have the right resolution back (which in my case is "like 3008x1692"). Very annoying.


The setup is MacBook Pro (13-inch, 2016, Four Thunderbolt 3 Ports) and of course LG 27UD88 screen connected with USB-C cable provided with the screen.
