If you're using iOS 10, the app might have gotten deleted my mistake. If this is the case, you can re-download it by going to the app store and searching for "music". The apple music app should show up as the first option. Once you've found it, just click the "Get" or download button (the cloud with an arrow) and it will install on your phone.
