iPhone, without being touched, will open apps, websites and make phone calls. What could be causing this?

It may have been a software issue in your case. That doesn't make it the most likely cause in all cases, however. But, doing back up and restore to factory state before taking the phone to Apple is always a good idea.
The fact that it took two factory resets is indicative that something else may be going on.
That was happening to me and i contacted Apple about it as well as VZ. In essence, they told me it sounded like a software issue. If it got worse after an update, it is because the update didn't update properly. They will ask you to back up the device and restore it to factory defaults. Then perform updates and add back your apps. If issue persists, factory reset and no apps to see if can duplicate. Then add back apps one at a time to isolate which app is the culprit.
It can also be either a bad cable or an counterfeit cable but that's unlikely. A bad digitizer is also unlikely.
I should have mentioned that after 2 factory resets the issue went away.

Both of those are more likely causes of what the OP is seeing than a software issue.
If it only happens when the phone is connected to power your cable is bad.
If it happens all the time the digitizer in your phone has failed. Make a genius bar appointment to get it evaluated.
