The problem is that quite often my Dock icons just show the default icon instead of its own.
For example here is my Dock and currently the System Preferences missing its own icon. I get this error regularly and usually killing the Dock from Terminal works. This bug is very annoying and I'd really like to find a proper solution for it.

By mistake duplicates /Aliases could be created for default applications .
One is permanently residing in Finder > Applications and other in the dock .
I don't see how is this relevant to my problem 😟
