Numbers Input form in shared document

Is there a way to keep Input forms in Numbers document when sharing via iCloud? Currently I can add Input form when creating new document on iPhone, (although it is not visible when editing in icloud.com), but it vanishes as soon as I share the document via iCloud. Am I doing something wrong?
