Every time I plug in my headphones to listen to my music, they work ok for about a minute or two and then the volume will decrease for no reason at all on the phone. I turn it back up and watch it go back down without touching anything. I can't use my headphones to listen to my music, it won't stay turned up. Anyone else have this issue?

Sounds like a bad volume button on the headphones. Try another pair, or make an appointment at the genius bar. The headphones are covered by the same warranty as the phone.

If they are defective and covered under warranty, then yes. Apple will replace them. If they have been physically damaged, you'll have to buy another pair.
