I have an iPhone 5S and recently I replaced the OEM charger with a more powerful 12W model.
I see that my phone now charges much faster suggesting that Apple has short changed users with an adequate charger, yo
so which models of phone can leverage the full 12W chargers that iPads use etc?

Only the iPhone 6/6+ and later are capable of the faster charge using the 12W adapter. However, the trade off is that it's larger. So, if Apple included it, there would be people complaining about how could Apple expect them to carry around this HUGE charger. Everything is a trade off.
The models you mention will only allow a 1 amp charging rate to protect the device. The device itself regulates this, not the charger.

iPhone 5w: 5 watts / 1.0 amp
iPad 10w: 10 watts / 2.1 amps
iPad 12w: 12 watts / 2.4 amps
The new charger I have is 2.4A at 5V makes it 12W
I installed iOS 11.0.1 and when I checked to see how my battery was, it was almost 100% after a measly 2 hours from DOA.
Clearly this charger works way better.
My 5S takes advantage, but my old 4 does not like more than 5W so it does not charge any faster.
I wonder of the 5, 5C can leverage 12W of power to fast charge the phones?
