Missing Table Tool in Notes app on iOS 11

I have a Notes app issue on my iPad 2017 running iOS 11. The toolbar that contains the table button is not displayed and the "+" button that makes that toolbar visible above the keyboard is nowhere to be found.

I don't have the issue on an iPhone 6 or iPhone 7. Upgrading to iOS 11.0.1 or 11.0.2 did not remedy the issue. Third party keyboards were not configured (and now I have even deleted them from the device). If I produce a table on another device I can edit it on the device with the issue, but there is no apparent way to display the toolbar that contains the table button and, therefore, no way to add a new table to a note on the device with the issue. I do have the little "+" button that brings up the "Scan Documents" function (thanks Apple for multiple "+" buttons in the same app that do different things...)

It is not Shortcuts: ON... not for me.

Check where the main account for Notes is set. If it is Your Google account or other... it will not work. You can only create scanned notes in iCloud section of Notes App. I switched to iCloud and the buttons magically appeared above the keyboard.
